چکیده

The aim of this work is to report the histopathological features of jaw bone in 24 cases of bisphoshonate-related osteonecrosis of the jaw. In all cases, lamellar bone trabeculae were thicker (bone sclerosis), had no osteocytes in the lacunae (bone necrosis), and exhibited a Pagetlike structure with marked signs of bone remodeling and formation of multiple trabecular compartments that were not connected to the trabecular surface. The Paget-like trabecular structure would result in a loss of trabecular vitality and in the onset of an inflammatory process due to microbial invasion. Bisphosphonate-related osteonecrosis of the jaw would initially be an aseptic process and osteomyelitis would develop after microbial infection. The results obtained from this series of cases would seem to confirm that bisphosphonates cause Paget-like bone regeneration, which would explain the likely aetiopathogenic mechanism and the association between bisphosphonate administration and the development of necrosis with secondary infection.

Jaw Osteonecrosis in Patients Receiving Oral Bisphosphonates Therapy

We describe the cases of three patients, under the care of the rheumatology service, who presented with osteonecrosis of the jaw whist on oral bisphosphonate therapy. The first case is of a 74-year-old woman with a 12 year history of sero-negative inflammatory arthritis, having been on oral steroids for 11 years, Methotrexate for the preceding 6 years, and oral bisphosphonates for 9 years. Clin...

Bisphosphonates and Osteonecrosis of the Jaw.

BACKGROUND Since 2003, reports have been published on necrosis of the jaw bones possibly being associated with the administration of bisphosphonates. Bisphosphonates are highly active inhibitors of osteoclasts which have been used prophylactically or symptomatically in the treatment of plasmocytoma, bone metastasis of malignant disease, tumor-associated hypercalcaemia and in the treatment of os...

Bisphosphonate Related Osteonecrosis of the Jaw: A Study of 18 Cases Associated with Fungal Infection

Osteonecrosis of the jaw (ONJ) is a serious complication associated with oral and intravenous bisphosphonate therapy. Its pathogenesis is not well understood and its management is difficult. Microbiological investigations have detected a variety of oral pathogens such as Actinomyces, Enterococcus, Candida albicans, Aspergillus, Haemophilus influenzae, alpha-hemolytic streptococci, Lactobacillus...

Denosumab-related Osteonecrosis of the Jaws: a Case Report

Bisphosphonates are potent and effective drugs frequently used to prevent the skeletal complications associated with postmenopausal osteoporosis in women, to manage patients with multiple myeloma, hypercalcemia, and metastasis of cancer to the bone. The search for new medications with the same therapeutic effectiveness as the bisphosphonates but fewer side effects has resulted in the discovery ...

Avascular mandibular osteonecrosis in association with bisphosphonate therapy: a report on four patients.

UNLABELLED Over the past three years, several reports have been published on jaw osteonecrosis possibly being associated with the administration of bisphosphonates. Bisphosphonates are highly active inhibitors of osteoclasts. These drugs are used for the treatment of multiple myeloma, bone resorption in the case of metastatic malignant diseases, tumor-associated hypercalcaemia, and in the treat...
